About ACT Endowment

Designating ACT Endowment as a beneficiary—Simply follow these steps to leave a gift beyond your lifetime:

1. Check your account statement for the phone number and website of your account administrator.
2. Request or download a beneficiary designation form.
3. Designate the ACT Endowment as a partial, contingent, or sole beneficiary of any of the following types of accounts you may have:
   • Retirement Accounts
   • Life Insurance Policies
   • Bank or Brokerage Accounts
   • Certificates of Deposit
   • Donor Advised Funds
4. Then, simply sign and return the form to your plan administrator and keep a copy for your files.

Naming the ACT Endowment in your will—Consider leaving a gift to the ACT Endowment in your will or trust.

The gift costs you nothing now but allows you to make a significant impact on the future. You can take care of family first and name the ACT Endowment to receive a specific amount or percentage of your estate. To leave a tax-deductible gift in your will or trust, please use the following language:

“I hereby give and bequeath $_____, or _____percent of the residue of my estate to the American College of Toxicology Endowment Fund, a non-profit organization exempt from Federal taxation under Section 501(c)(3) of the Internal Revenue Code, Federal tax ID number 36-3007817.”
